Waiver Forms Online

Here are the waiver forms that you need to attend the gym. You can get them at the gym or now you can print them off online. The first one is the waiver form to be able to attend the gym and the second one is the mandatory one time $10 insurance fee form. You need to fill out both forms and bring them in. The files are in .pdf and you will need acrobat reader or a program that can read .pdf. I suggest using foxit reader or you can get acrobat reader here.
